How to Enroll


IAFF members who responded to the September 11th terrorist attacks at the WTC, Pentagon, and Shanksville PA crash sites are eligible to enroll in the WTC Health Program. Check the detailed eligibility descriptions here.





You will need to submit documentation showing:
  • Your work or volunteer activity at the World Trade Center site, the Pentagon, or Shanksville, PA
  • Your location at the World Trade Center site, the Pentagon, or Shankville, PA
  • The time period that you worked or volunteered at the site
Click here for examples on the types of documents that are accepted. If you cannot any of this supporting documentation, you must write a description of what you did to try and get the documentation why you were unable to obtain any information.

If you need applications in languages other than English, you can download them from the WTC Health Program website: http://www.cdc.gov/wtc/apply.html





Follow the instructions on the form. You can fill out the form electronically or print out the form and complete it manually. If you complete the form electronically, you will need to print the form and sign and date it before sending your application. Your application will be processed as soon as it is received. WTC Health Program staff will contact you if you need to provide additional supporting documentation. You will receive a letter notifying you that your application was accepted. If your application is not accepted, the letter will explain how you can appeal the decision.
